Biographical Sketch:Dr. Subramanian is a General internist and Geriatrician with advanced training in epidemiology and health services research from the Indiana University School of Medicine. She is considered an expert in Medical Home models for the Frail Elderly to address Geriatric syndromes such as Dementia to prevent functional decline and improve independence among older adults. She is the Medical Director for VA-GRACE [Geriatrics Resource for Assessment and Care of Elders] to involve seniors and their primary care physicians in a program to optimize health and functional status, and to decrease high-cost emergency department visits and hospital admissions. She has since served as a National Primary Care representative for the VA Dementia Steering Committee and as Chair, Diabetes and Aging Work Group, and on the SGIM Geriatrics Task force.

Research Interests:Research that develops and tests new healthcare models for frail older adults and with chronic illnesses such as heart failure and diabetes to improve health while lowering costs.
