Referral Options:

1. Referral for Outpatient Education
   
   
   • Patient will be scheduled to come to the clinic for a comprehensive education session that includes diet, medication compliance, weight and blood pressure monitoring, disease-specific information and other components based on patient needs.
     
     
   • Follow-up education will be scheduled as needed.
     
     
2. Referral to Advanced Practice Nurse
   
   
   • Patient will be scheduled to see an advanced practice nurse in the clinic for assessment and plan development. An educational component is included.
     
     
   • Patient medications can be started and/or titrated based on the evidence-based protocols of the clinic.
     
     
   • Patient care plan created in collaboration with the referring physician.
     
     
3. Referral to a Heart Failure Specialist Cardiologist
   
   
   • Appointment with a cardiologist that provides care through the clinic.
     
     
   • If no preference, schedule with next available appointment on a rotating basis.
     
     
   • If patient or physician has a preference, schedule with that physician.
     
     
4. Referral for Ventricular Assist Device Placement
